We believe having a good sleep is the cornerstone of living a healthy life. Access to sleep medicine should be easy and affordable. It is time to prevent many chronic diseases like hypertension, diabetes, obesity, heart attack and stroke by promoting the essential role of a good sleep through screening for sleep disorders and public education about the consequences of poor sleep habits and sleep disorders. Be prepared for your first visit

At your first visit, we need to obtain a comprehensive sleep sleep-related history, to save your time, please go to the "Forms" download, complete the following forms and email it to us at least two weeks in advance of your appointment time.

1- Epworth sleepiness scale (For all patients)

2-RLS rating score (For patients with a restless leg problem)

3-Insomnia severity index and 4- Stanford Insomnia Questionnaire (For patients with a complaint of insomnia)
